The title is all you need. Add the author if you want it on the printed list.

One book per line. To add the author, put a dash after the title, like Charlotte's Web - E. B. White. A list copied from a note on your phone or a spreadsheet works too.
Every book on this list gets the same date. If you don't know the day, the last day of the month you read them is fine. You can change any book afterward.
